WHAT IS THE DIFFERENCE?


The basal implantation technique can be summarized in five points:

  1. All patients who request the implants can be treated, without exception, regardless of the extent of bone loss or the presence of absence of residual teeth.
  2. Only a single surgical appointment is required. Basal implants are loaded and splinted immediately or early using long-term provisional bridges. In some cases it is possible to provide the definitive bridge immediately, in other cases the first bridge serves as a long-term temporary.
  3. The low number of surgical and other treatment steps reduces overall cost. Rapid and effective measures can be taken should complications occur.
  4. Basal implants hardly interfere with the blood supply to the host bone, at any rate much less than comparable conventional implants.
  5. Because basal implants can be immediately loaded, patients can quickly resume their normal lives.



Before the advent of basal implants, all patients with an insufficient on supply for immediate placement were forced to undergo the following extensive treatment.
 
Removal of all teeth (if required); removal of all granulation tissue, cysts and other undesirable residue from the bone.

Bone augmentation approximately 2 months later (with potential problems not unlikely to occur at the donor site), followed by a 3- 6-month waiting period
Insertion of axial implants (screw implants)

Finally, after another 3- 6 months, insertion of the bridge or other prosthetic superstructure.

In addition to this already tedious treatment plan, soft-tissue corrective measures frequency have often to be taken. Surgical risks accumulate throughout this four-step treatment phase: Despite all efforts and despite all the published claims, the clinical success of dental implantological treatments becomes unpredictable as soon as bone augmentation is required.


The conventional axial implant procedure created a paradoxical situation for most patients: For many months they were forced to do exactly what they were trying so hard to avoid, namely to wear a removable denture. And in many cases things were even worse: There were often times when not even the removable denture could be worn, specifically after the bone augmentation procedure and/or following the insertion of the implants, during the first phase of the waiting period.

"Basal Implants" - No more bone grafting


                                          WHAT IS “BASAL IMPLANTOLOGY”?

The term “basal implant” was brought into our profession approximately in 1998. It reflected the idea and the principle, that the load transmitting surfaces of implants should be positioned in basal bone areas, because these areas are stable and the bone there is not prone to atrophy. The bone that survives everything and is present after many edentulous years was utilized.

“Basal bone” is the bone which will not be resorbed throughout life, it is structurally needed. Typically it is not the alveolar bone created during teeth eruption and for teeth. Note however that crestal cortical bone resorbs or re-locates during the resorbtion process which is called atrophy.

Another aspect of basal implantology is the usage of cortical bone areas. Cortical bone should be used for implant anchorage for good reasons: bones typically consist out of a strong, highly mineralized outer cortical and an inner portion of bone, called “spongious bone”. While cortical bone areas are structurally needed and always repaired, spongious bone areas are (from the bones mechanical point of view) not needed.

- One reason why implantologists should preferably utilize corticals, is that cortical defects created during the implant surgery are always repaired. This promotes/guarantees the implants integration.

- Another reason is, that cortical bone is highly resistant due to its high mineralization, allowing immediate loading protocols.

In the last decade the combined usage of basal and cortical bone areas with the help of BOI® /TOI ® implants and elastically designed basal screw implants (BCS®, GBC ®) has been introduced in dental profession.

Hence the old term “basal implantology” does not cover all principles behind this concept. We better describe the technology today as “cortico-basal implantology”.

Today elastic basal screw designs are used in combination with lateral basal implants. Basal implantology has made implantology free from the ties of the mandible and the maxilla, it works independently from bone being present in those bones. Today we utilize bone areas of the maxilla, the mandible, the sphenoid bone and the zygomatic bone.


What once has started with “BOI®”, has become a movement based on deep surgical knowledge, on the thorough knowledge of the bone`s properties, and on consequent application of a prosthetical protocol.

          Immediate Loading- ie; patient can be given crowns / bridges the very next day.
          Delayed loading- very often a waiting period of minimum 3 months is necessary after loading the implant with prosthesis.
          Much cost effective when compared with 2 or 3 piece implants.
          Expensive with respective to cost of implants as well as time taken for treatment procedures.
          From patient point of view, less complex treatment procedure, less number of sittings, crowns / bridges can be cemented on a day or two.
          From patient point of view, more number of sittings, Crowns/ bridges cemented only after 3 months after healing phase. Expensive.
         There is no screw loosening since there is no separate abutment-screw-implant assembly.
          Screw loosening is very common. Being 2 piece, the relation between root portions and abutment can present many problems.
          Maintenance is very simple. Being 1 piece, strength provided by implant is excellent and there is no separate root portion and abutment portion.
          Maintenance is more complex. Very often screw has to be tightened at periodic intervals as there will be micro movement between implant and abutment.
